What a driver can do while the car is moving
A driver has a much smaller set of options than a passenger, and the reason is not space. It is that everything must cost zero attention, keep both hands available to the wheel, and keep both eyes forward. That rules out most of what gets written about movement in a seat, and what survives is worth knowing precisely, because on a long leg of motorway there is nothing else.
The rest of it — anything that needs a hand, a look down, or a shifted foot — belongs to the minutes when the car is stationary and the engine is off. Those are a different subject and this post keeps them separate on purpose.
What driving does that ordinary sitting doesn’t
Your right leg works and your left leg doesn’t. In an automatic, one foot moves between two pedals in a small arc while the other sits idle on a floor pan. In a manual the left foot works, but only in one narrow pattern. Either way the two legs spend the journey doing different jobs, so by the end one hip and one side of the low back have had a different day from the other. The asymmetry is the thing that distinguishes driving from being flown somewhere.
The seat and the belt decide your pelvis for you. Car seats have deep bolsters and a pan that tilts back, and the lap belt sits across the front of the pelvis. Together they hold you in one pelvic position more firmly than an aircraft seat does, and the belt makes big adjustments feel like a fight. Small ones still work, and they are the whole game here.
Your arms are held out in front of you, above waist height, for hours. That is a sustained low-level demand on the front of the shoulders and the upper back, and it is why the shoulder-and- neck fatigue on a long drive arrives before the back fatigue does.
Your head barely rotates. You check mirrors, which means your eyes move and your neck mostly doesn’t. Neck rotation is one of the ranges that complains fastest about being unused, and you can’t address it by turning your head to look at things — the road needs your face pointing at it.
Vibration. Continuous, low-frequency, coming up through the seat. It is a plausible part of why the tiredness after a long drive feels different from the tiredness after a train journey covering the same distance.

Everything above keeps both hands within reach of the wheel and both eyes on the road. Nothing above requires you to look at your body, reach for anything, or take a foot off a pedal you might need.
Pelvic tilts, adapted for a belt
The same movement that works in an aircraft seat works here, but the belt changes how it feels, so it’s worth describing again.
Sitting normally, roll the top of your pelvis forward so a small arch appears in your lower back and you feel yourself moving onto the front of the sit bones. The lap belt will resist this slightly; don’t push through it, just go as far as it lets you go easily. Then roll the other way, tucking, so the lower back flattens and you settle behind the sit bones. Move slowly between the two.
Stop when the two ends stop feeling different from each other. That’s the signal that the tissue has its range back for now, and it is a better guide than a count, because how long it takes depends entirely on how long you have already been sitting. Early in a drive it happens almost immediately. Five hours in it takes noticeably longer, which is itself information.
It costs no attention and addresses the low, central ache that is the commonest complaint on a long drive — the same mechanism described in a confined seat you can’t leave, with a belt added.
Changing where your hands are
Most drivers have one hand position and hold it for the entire journey. Moving them is not about grip comfort — it changes the angle at the shoulder, the rotation of the forearm, and how much the upper back is asked to hold, all at once. It is the largest change available to a driver and it is free.
On a straight, clear road, move both hands to a different part of the rim. Lower is generally less demanding on the shoulders than higher; a wider spacing loads differently from a narrow one. Keep both hands on the wheel throughout; this is a change of position, not a release.
Change again when you notice you have stopped noticing your arms. Sustained holding stops reporting itself after a while, which is exactly when it is accumulating. The moment you become aware of your shoulders is later than the ideal moment to move.
Scapular squeezes, and the seat as a reference
Draw your shoulder blades back and slightly down until you feel them make contact with the seat back, hold that contact, then release. The seat gives you something the same movement lacks when standing — a surface that tells you unambiguously whether the movement happened.
Repeat until the contact stops feeling like an effort to reach. Then leave it. It is invisible to passengers, it needs no hand and no glance, and it directly opposes the forward-shoulder position that holding a wheel produces.
The left leg, and only in an automatic
If there is no clutch pedal, your left foot is doing nothing for the entire journey, and it is the only limb with genuine spare capacity.
With the foot flat on the floor and well away from the brake, lift the heel keeping the toes down, lower it, then lift the toes keeping the heel down. Slowly. It works the calf and moves the ankle through a range it has otherwise spent hours not using.
In a manual car, do not do this. The left foot’s job is the clutch and it needs to be where the clutch is, immediately, without a decision. This is a case where the constraint simply removes the option, and the honest answer is that a manual driver’s left leg gets nothing until the car is stopped.
What has to wait until you are parked
Everything else. Any twist, any reach, any movement of the seat, anything involving getting a hand onto a leg, anything you have to look at. Those are worth doing and they are worth doing properly, which is why they have their own post: the stop itself is the only real break a long drive contains, and treating it as a fuel transaction wastes the one opportunity in several hours.
What this doesn’t replace
Nothing on this list is comparable to standing up. It stops a five-hour leg being five hours in one unvarying geometry, and that difference is real and modest. The unconstrained version — out of the car, weight on your feet, spine free to move — beats all of it, and the reason this post exists is that on a motorway with the next services a long way off, it is not currently on offer.
When it isn’t the ordinary kind
Numbness or pins and needles in a leg or foot while driving, pain that travels down one leg, or any loss of confidence in what a foot is doing on a pedal are reasons to stop driving as soon as it is safe to do so, and to ask a clinician rather than to adjust your seat. New or one-sided calf pain or swelling, chest pain, or breathlessness during or after a long journey need urgent medical attention.
Drowsiness is outside this post’s subject entirely, and no amount of moving in the seat is an answer to it.
