Working with your arms above your head

Almost everything written about neck and shoulder discomfort assumes the problem is a head tipped forward and arms held low. Overhead work is the opposite situation and it needs the opposite answers. Painters, electricians, plasterers, hairdressers, mechanics under a lift and anyone stacking a top shelf spend a shift with their arms above shoulder height and their head tipped back, and the standard advice actively points the wrong way.

The constraint: the work is where it is, the ceiling does not come down, and the position is set by the task rather than by preference.

What overhead work actually loads

Your neck is held in extension. Looking up at what your hands are doing compresses the structures at the back of the neck, and unlike a forward head position it usually comes with the jaw and throat stretched too. The complaint at the end of the day is often at the base of the skull rather than in the shoulders.

Your shoulders hold a position at the top of their range. Arms above shoulder height is a demanding place to sustain, because it is near the end of the available movement rather than in the middle. Muscle working near the end of its range fatigues faster and gives less warning.

Your arms are working against gravity with nothing supporting them. No surface, no rest, and often a tool in one hand adding to it.

Your low back arches to help. This is the part people miss. Reaching up is easier if you tip your pelvis forward and arch the low back, and everyone does it without deciding to. That is why overhead work produces low-back ache in people who are not lifting anything.

One arm does more. Tool in one hand, steadying with the other. The dominant side takes the shift.

You are on your toes, or on a ladder. Which adds a calf and balance component on top.

The last item is the exception on this site: usually the constraint is fixed and unarguable. With overhead work it is sometimes genuinely adjustable, and where it is, it beats everything else.

Looking down, which is the whole point

The single most valuable movement for overhead work is the one nobody writes about, because all the writing addresses the opposite problem.

Between tasks, let your head come down slowly so your chin moves toward your chest, and stay there. Not pulled with a hand. Just let the weight of the head do it. You will feel it along the back of the neck, and after a stretch of overhead work it usually feels like relief rather than a stretch.

Stay until the sensation stops being pronounced. Then come up slowly — coming up fast from neck flexion after a period of extension is unnecessarily abrupt.

Do this frequently and briefly rather than occasionally and at length. A few seconds of the opposite, repeatedly, is what stops the extension accumulating.

Letting the arms hang, properly

Arms fully down, shoulders released, hands loose. That sounds like doing nothing and it is not: most people finishing a stretch of overhead work keep their shoulders elevated even after their arms come down, because the muscles that lifted them have not switched off.

Let the shoulders drop. Then drop them further — there is usually more available than you first find. Roll them slowly backward a few times to find the bottom.

The check: can you feel the weight of your arms hanging from your shoulders? If you cannot, the shoulders are still holding, and that holding is the fatigue you will feel tonight.

The tailbone tuck, because your low back has been arching

Reaching up produces a forward pelvic tip and an arched low back, every time, in everyone.

Standing normally, gently tuck the tailbone under so the low back flattens slightly, then release. Repeat until the flattening happens without effort.

Better still, learn to do it while you are working. Reaching overhead with the tailbone gently tucked means the reach comes from the shoulder and the upper back rather than from the low back, and it is the single technical change that makes a day of overhead work cost less. It will feel like less reach is available at first, because it is — the arch was borrowing reach from the spine.

That is the same movement used for the opposite purpose in a post you can’t leave, where the arch develops from standing tired rather than from reaching.

The forward hinge, which puts the whole back the other way

Bigger than the tuck and worth doing when there is a moment.

Feet apart, hands on your thighs, hinge forward from the hips — sending the hips back rather than rounding the spine — until your torso is angled down and your hands are taking some weight on your legs. Let your head hang. The whole back is now the opposite of the position it has been holding.

Come up when you have stopped feeling the relief and started feeling the effort in your legs, and come up by pushing your hips forward rather than by hauling your torso up.

Do not do this on a ladder, on a step, or anywhere that a change in your balance matters. Feet on the ground, nothing in your hands.

Changing hands, and changing the work height

The tool hand. If the task allows either hand, alternate. The non-dominant side will be slower and worse, which is precisely the point — the dominant side has done every hour of every shift. Change on a task boundary rather than when the arm tires, because waiting for fatigue means the same side is always fresh.

Raising the work or lowering yourself. This site normally refuses to suggest removing the constraint, because the whole audience is people whose situation cannot be changed. Overhead work is the one place where it is often genuinely available and routinely overlooked: a taller platform, a step, working from a chair instead of standing under it, laying the item down and doing the fiddly part at bench height, or bringing what you are stacking down to a trolley rather than reaching up to it.

If it is available, take it, because the position is the whole problem and nothing on this page competes with not being in it. If it is genuinely not available, the rest of the page is what there is.

What this doesn’t replace

Nothing here makes a day of overhead work not a day of overhead work. It interrupts the sustained position and it reverses the direction the neck and low back have been held in, and both of those matter. Neither of them substitutes for a shorter task or a better height.

Shoulder work near the end of its range is a real load and it is worth taking seriously rather than treating as ordinary stiffness. The related standing-shift problems — feet, calves, hard floors — are in standing all day is its own problem and apply on top of this.

When it isn’t the ordinary kind

A shoulder that has become genuinely unable to reach overhead, rather than reluctant, is a clinician’s question and not something to work through. So is pain that appears at a specific point in a movement every time, pain that wakes you at night, or pain, numbness or weakness travelling down an arm into the hand.

Dizziness or visual disturbance when you tip your head back is a reason to stop and to ask a doctor, not a stiffness to stretch out.

Falls from height are the actual serious risk in overhead trade work, and nothing on this page should be done while you are on a ladder or a platform.
